The Affordable Care Act's Senior Care Benefits: Medicare Parts B and C

Obamacare -- the Patient Protection and Affordable Care Act ('ACA') -- has done a huge amount to improve senior care across the country, much of it through the expansion and modification of Medicare. Among the many changes: the improvement to the efficiency and effectiveness of Medicare has kept the Medicare Part B monthly premiums down. In 2013, the premium was lower than analysts predicted -- a mere $104.97; in 2014, it stayed $104.97.
That's impressive in and of itself. But the ACA has done more than just keep the Part B costs down; it's also reduced the cost and improved the effectiveness of Medicare Part C; the 'Medicare Advantage' program that allows private insurance companies to supplement the existing Medicare benefits at lower-than-normal costs.
Medicare Part C
For most seniors, the only viable alternative to Medicare Parts A and B is Medicare Part C, which allows private insurance to accept money from Medicare to pay part of the premiums for senior citizens. Because the payments are shared between the government and the citizen, the citizen is able to afford an insurance package they otherwise couldn't -- which, for most seniors, means "an insurance package that actually covers most health care costs."
By law, Part C plans have always been required to provide at minimum the same coverage offered by Medicare Parts A and B, but historically, they've been the source of some of the most ingenious developments in the senior care field, including the concepts of 'case management' and 'care coordination.' Nearly 30% of all Medicare recipients take advantage (no pun intended) of Medicare part C.
How the ACA Has Improved a Great System
The big 'problem' with Medicare Part C has been the disparity between the amount that private insurance has paid a provider for a given service through Part C and the amount that Parts A/B paid for the same service. The payouts varied quite a bit, with some policies paying under but most over-paying for the same service, putting 'standard' Medicare recipients at a disadvantage in the eyes of the medical establishment.
So, the ACA has enacted regulations that keep the Part C payouts to insurance companies within 5% of 'standard Medicare' payouts, based on the 'customary and reasonable' charges for each service within the geographical area where the service was provided. They've also required that Medicare Part C plans be sold on the same healthcare exchanges as traditional insurance, which means that seniors are now able to compare Medicare Advantages plans side-by-side and see which are the most cost-effective and offer the best coverage.
The effects of that change have been that insurance companies profit slightly less from each Part C participant they serve, in part because per-service payouts are down and in part because of the price competition caused by the exchanges. However, they serve significantly more participants, because more seniors are aware of and comfortable with buying Part C when it appears on the ACA health exchanges. The net effect is to the insurance companies' benefit: while profits-per-enrollee are down about 10%, the number of enrollees is up about 33%, so the bottom line is still increasing with each passing year.

The Affordable Care Act's Senior Care Benefits: Nursing Home Consumer Protection

In addition to providing a significant amount of insight into the quality of nursing homes around the country, the Affordable Care Act ('Obamacare') also gives seniors a significant amount of protection from nursing home abuse.
Requirements for Nursing Facilities
The ACA created a requirement for every nursing facility to create a 'compliance and ethics' program that will be effective in preventing misbehavior of the criminal, civil, or administrative varieties within the facility. This includes creating a position for one or more individuals who have the express job of ensuring that the program is followed throughout the organization, and to whom all residents have the right to submit their complaints.
Furthermore, the program must offer substantial protection to any residents who do complain, to ensure that no one is retaliated against for the act of submitting a complaint.
Constant Improvement
Beyond merely allowing residents to report problems, the ACA also obligates every nursing home to set up an individual or department to be in charge of 'Quality Assurance and Performance Improvement.'
The Department of Health and Human Services keeps track of 'best practices' for nursing facilities on a nationwide level, promoting those systems that have been proven to be most effective at keeping patients healthy and happy. The QA/PI programs that each home are required to keep in place are given access to the full dataset of best practices, and work to ensure that their facility works toward implementing as many as make sense for that facility.
Criminal Screening
The ACA provides funding for the Center for Medicare and Medicaid Services to work with each state to create programs nationwide for the easy criminal screening of health-care workers, and it specifically provides money for nursing facilities and other long-term care facilities to perform that screening.
In addition, any nursing or long-term care facility that receives federal funding is obligated to report any crime against any resident to the Department of Health and Human Services, or they will lose those funds.
Nurse Training
Obamacare also provides those same nursing facilities with additional funds to prevent crime in the first place -- specifically, by paying facilities to train their nurses in Resident Abuse Prevention, as well as dementia care (because patients with dementia are the most common victims of nursing crime.)
Full Disclosure
Finally, the Affordable Care Act requires that every nursing facilities make public their owner, their funding sources, and their financial relationships. This prevents a particularly nasty scenario wherein nursing home owners who are caught abusing their patients simply move to a new state and start over.
The ACA has done an incredible amount for seniors who are entering nursing homes, most of which is far enough in the background that you'd never notice -- except that the number of nursing home horror stories is dropping every year.

If there's one nearly-universal truth about Obamacare -- the Affordable Care Act (ACA) -- it's that misinformation about it abounds, and most people don't actually understand what it is doing for them at all. Let's take a bit of time today to specifically dispel some tenacious myths about how the ACA is going to affect elderly Medicare recipients.
Myth: Medicare Recipients Must Now Buy Additional Insurance
There is a lot of language in the ACA and the talk about the ACA is how many people are going to have to buy new insurance. But if you're receiving Medicare already, you're covered. Medicare (and Medicare Advantage plans) already qualify for all of the ACA's new rules, so there's no need for you to do anything except reap the benefits of the new law.
Myth: Medicare Recipients Will Have to Change Doctors
This is best described as 'possibly partially true in a way,' actually. The deal is this: Medicare payouts to insurance companies with Medicare Part C clients are going down, so insurance companies are trying to find a way to absorb that decrease in income. One of the ways they may choose to do that is to shrink their networks -- which may mean that some recipients will find out that their doctor has recently been removed from their insurance company's network. This will mean that they have to change doctors. This isn't because of anything the ACA does legally, but is a possible indirect result of changes the ACA are making to the payout structure.
Myth: Medicare Premiums are Going Up Due to the ACA
Make no mistake: Medicare premiums are going up. But it's not because of the ACA. They go up every year, according to the same algorithm. The net effect of the ACA on those premiums is that they're going up more slowly than they would have otherwise -- so the ACA is actually of direct benefit to anyone that has to pay Medicare premiums.
Myth: Quality of Care is Going to Go Down under the ACA
Not at all -- in fact, the exact opposite is happening. Remember when we said just a moment ago that Medicare payouts to insurance companies are going down? Well, they're not going down as much for insurance companies that offer superior coverage. In other words, insurance companies actually have a financial incentive to give you better care than the basic Medicare standards (for the first time.) So quality of care is headed up, not down.
Myth: The ACA is taking $716 million out of Medicare
A complete misunderstanding -- the $716 million figure is the amount that the Congressional Budget Office calculated that Medicare spending would go down. Mostly because of the twice-aforementioned payout drops... which means the $716 million is not going into the pockets of insurance companies, which is actually effectively putting that money back into Medicare. The Affordable Care Act's Senior Care Benefits: Seniors Without Medicare

In general, if you're a senior citizen and you aren't using either Medicare or Medicare Advantage, you're in a fairly limited number of circumstances. In general you:
• Don't qualify for Medicare (usually because you haven't lived in the US for five consecutive years before turning 65),
• Already have insurance other than Medicare (usually in the form of retiree health benefits), or
• You have no health coverage (almost always because you missed the opportunity to apply for Medicare during the first 7 months of the year.)
Let's cover how the Affordable Care Act benefits each of these groups, working from the bottom up.
If You Have No Health Coverage
Obamacare will levy a tax penalty against you if you fail to acquire health insurance by the beginning of the next year. On one hand, that's not great -- but on the other hand, the penalty is generally smaller than the cost of acquiring health coverage, and on the other hand, having insurance is pretty darn important if you're 65 or more years old in the first place. (The penalty for not having insurance in 2015 is 2% of your yearly household income.)
Fortunately, the process for obtaining healthcare coverage under the ACA is pretty easy. Every state has a new health insurance marked known as an 'exchange' that is run by either the state or the Federal government. Each exchange offers a variety of insurances tailored to that state's laws and to the local healthcare economy.
So, in short, the primary benefit of Obamacare if you don't have health coverage is to make it easy for you to obtain insurance. As an added benefit, if you don't have health insurance because you can't afford it, you may qualify for a tax subsidy that will make a low-end health plan easy to afford!
If You Have Health Insurance Already
Seniors who already have health coverage that qualifies for Obamacare's restrictions (which, by now, 100% of all insurance plans should) benefit less directly from the ACA, but they do benefit.
• Coverage Doesn't End for 'Essential Benefits': Preventive care, hospital visits, chronic disease care, and a few other 'essential benefits' cannot be capped by your insurance policy anymore.
• You Don't Pay More for Pre-Existing Conditions: Having a disease when you purchased your insurance used to mean you either wouldn't get insurance, or you'd pay through the nose for your insurance. No longer, thanks to the ACA.
• Free Preventive Care Benefits: The ACA ensures that all health insurance must provide a wide array of annual preventive care benefits at no cost to you.
If You Don't Qualify for Medicare
If you don't qualify for Medicare for any reason, but you are over the age of 65, you are treated exactly like anyone else who has no medical coverage (above.) But there is a critically important note that needs to be made here: be absolutely certain that you don't qualify for Medicare before you go shopping on the exchanges -- because if you do qualify, it's a crime for anyone to sell you a non-Medicare health plan.

After gaining independence, Mozambique was hailed for its efforts to establish a health care system that would have been the envy of Southern Africa. Unfortunately this was one of the systems targeted during the civil war and the countries health system has never fully recovered since.
The current system is two pronged with provision for basic healthcare provided by the government and private care available through privatized companies. The variance between these two is astronomical and the two systems are almost not able to be compared.
Government health
The Mozambican government is responsible for the provision of basic healthcare which is supposed to be provided through an infrastructure at national, provincial and district level. A large number of issues are faced by this system with the biggest being woefully inadequate number of staff, chronic underfunding and accessibility. The rural areas of the country have little or no access and a disproportionate number of patients to doctors, while the urban areas have a high number of infectious cases.
The health system is under constant strain and has become reliant on foreign aid and workers to bolster the system and provide what little care it can.
Private health providers
This has been a part of the healthcare industry that has not really seen any developments until lately. Traditionally those who were able to afford proper medical care have travelled to neighboring South Africa for such services. This trend is slowly being reversed and the first private hospital opened in the capital Maputo in 2012. Money Can't Buy Fitness, You Gotta Earn It

It's long been said that money can't buy everything and most will agree if they are truly being honest throwing silliness aside. Money can't buy you true love. Money can't buy back your youth or bring back loved ones that have passed on. And additionally, money can't buy fitness no matter how much you shell out of your pocket.
To make that point clear, all the money in the world cannot automatically create that perfect physique regardless of how much you are willing to pay. Sure money can pay for a private gym membership or even allow you to build your own gym in your backyard. Money will also allow you to hire a 24 hour a day personal trainer along with a chef to be at your beckoning call whenever you need a healthy meal prepared.
Money can also pay for weight reduction surgeries or "tummy tucks" which can give the appearance of a reduction in weight. However even in this case the person is not fit, they just have less fat - completely different case.
However even with these things that money can potentially buy, they will not magically get you fit. The sweat and effort will still have to come from you and that is something money cannot buy.
You have to earn fitness and that is the beauty of it. No matter how rich or poor a person is, fitness is something that judges not on how financially well off you are.

Don't forget about the importance of tying healthy nutrition into your fitness plan so your weight loss results will be amplified even more so. Once you are able to combine both of these healthy habits regularly, losing weight and getting fit will come much more rapidly than you ever thought possible.
